This Privacy Policy is a general website and consumer privacy notice. It is not a Notice of Privacy Practices under the Health Insurance Portability and Accountability Act of 1996 (HIPAA). If 1st Alliance is required to provide a separate HIPAA Notice of Privacy Practices for particular services or information, that notice will govern our use and disclosure of protected health information for those activities.

  • Transaction and service information, such as products viewed, requested, purchased, or rented; delivery or pickup details; rental dates; equipment preferences; payment status; and customer-service history.

  • Health and equipment-related information that you choose to provide, such as mobility needs, height or weight when needed for safe product selection, equipment specifications, prescriptions, provider orders, diagnoses or functional limitations relevant to a request, and discharge or recovery information.

  • Referral and provider information, such as the referring organization, health care professional, facility, contact person, and supporting documents.

  • Payment and billing information. Payment card information may be collected and processed by our payment processor. We may receive limited transaction details, such as payment status, card type, and the last four digits of a card, but we do not intend to store full payment-card numbers on our own systems.

  • Communications and submissions, including contact-form messages, emails, telephone calls, voicemail messages, reviews, survey responses, uploaded documents, and requests for support.
